
About Pathways
Pathways is a service initiated and delivered by Praxis Institute for Participatory Practices for organisations working at grassroot levels. It aims to ensure that the communities at risk receive integrated care. It is an innovative tool that makes case management easier by maintaining their case records in one place and streamlines support provided by organisations to persons vulnerable to various kinds of risks and vulnerabilities. It provides a platform for generation of reports helping grassroots organisation analyse their own data and further use it for purposes like engaging with stakeholders, planning, improved case management and monitoring. Strict privacy and data encryption controls protect victims’ identity and offer security far beyond paper-based systems.

INTAKE
Prepare profile of the client and others associated with a case

List out needs identified in the assessment process and determine steps for addressing them

Regularly monitor the status of a case, identifying gaps and further steps to be taken

Regularly check whether a client and his/her family are receiving appropriate services, reviewing the plans accordingly
JOURNEY OF PATHWAYS
Pathways is a digital case management system for vulnerable communities. It will support partner organisations in providing integrated care to communities at risk.
The team members visited different pilot partners to develop a better understanding on case management, their current format to maintain records and challenges they are facing.
After rounds of meetings with the stakeholders a final structure was evolved. Partners were oriented on functioning and features of software. A live demo was conducted after which the software was rolled out.

Onboarding partner NGO
Kick-off meeting was conducted with several NGOs including the pilot partners. This meeting helped understand their current challenges and expectations.
Expert panel review and designing
Meetings were conducted with community members, ground level staff and other experts. These meetings helped us to review the framework of Pathways and fine tune it as per their experiences .
